  • Please use the applicant's FULL legal name - no initials. If there is no middle name, enter "NMN".

  • Please do NOT use ALL CAPS.

  • Be sure to enter the date at the bottom or the application will not submit.

  • It is your responsibility to verify that we are in receipt of your completed application. You are welcome to contact our front office prior to the deadline to make sure your application has been received. A completed application will include you in the lottery. You will receive a confirmation email upon successfully submitting this form.

Completed application must be filed with the Parker School by February 1, 2023.
Prospective student & parent are encouraged to attend one of the Information Sessions.
