Parent OPT OUT Form for
Sharing of Family Directory Information
Our school is sometimes asked to share family directory information to outside organizations. Directory information is a type of student data that schools can disclose to third parties without explicit consent from the student or their parents. It generally includes information that would not be considered harmful or an invasion of privacy if shared. Examples of directory information typically include name, address, phone number, email address, and dates of attendance. While directory information can be shared, it's still subject to certain restrictions and cannot be used for commercial purposes without consent.
The following is the Parent/Guardian Opt Out Form for Family Directory Information. If you wish to OPT OUT and have the school NOT share your directory information with outside organizations, you MUST complete and submit the form below. To opt out means that a parent, guardian, or eligible student is denying permission for the school to share their directory information with outside organizations.
To make your request known, you must put your request in writing to the head of school (Brian Harrigan - [email protected]) OR by completing the form below. If you do not express your preference, you are giving permission to share your directory information.
